How Do I Upload the Completed Form? (Verified Skills)Updated a year ago

Once the skills form has been completed by your evaluator, sign into your account and return to your skills course, click Modules on the left-hand side, and navigate to the second Hands-On Skills Check page.  Click the orange Start Assignment button at the top of the page.



Scroll down to access the File Upload area, click Upload File, click Choose File and click Submit Assignment.  Select your completed skills form on your device and click Next.  


Once your form has been received, we will send you a confirmation email, and our team will review and verify your form for both successful completion and to confirm your evaluator’s status and participation. We also suggest informing your evaluator to expect an email from us. If we do reach out to them as part of our verification, a quick response will help expedite the process. A typical turnaround time is three to five business days. 

After we have verified and approved your submission, this step will be graded as “pass” in the course, and the course will be marked complete. At that time, we will also email you with instructions for downloading your updated provider card that indicates you have completed both didactic and psychomotor skills evaluations. 


*Please note that ACLS.com does not provide in-person or virtual hands-on skills check services. The skills evaluation course, including all forms, instructions, and verification steps, is only meant to assist you with the process. To complete this step, you must find a qualified instructor to conduct the evaluation. We recommend sharing the evaluator instructions from your skills course with your instructor for a seamless process.
